Trips – Forms & Docs

Forms are a necessary part of any trip, but they can also be a logistical nightmare as you seek to coordinate multiple students and their parents. To that end, we’ve tried to make it as easy as possible for you to access everything we require.

Participant Release Packet

This release packet covers participant basic information and general release statements that are normally associated with a trip permission form. Every participant (including adults) must have this filled out in order to be on the trip.

Our system will send the appropriate form to your students and adult leaders, send you and your participants important reminders, and collect signatures.

Leader’s Covenant

This is a written agreement made between the LeaderTreks staff and your adult volunteers/sponsors on the trip to partner together for the development of the students. This must be signed by every adult coming on the trip.

It is essential that you and all of the adult staff on your trip carefully read and sign the Leader’s Covenant. This document outlines the cooperation LeaderTreks is agreeing to and asking of you and your staff during the trip. If you have any questions at all about this form, please ask your trip leader for clarification.

Standard of Conduct

This outlines rules and standards associated with a LeaderTreks trip. It is a requirement that you walk through these standards with every participant on the trip. This was also signed as part of the contract.

Our Standard of Conduct document describes the expected behavior of students while on a LeaderTreks trip. Please read this form and go over it with your students prior to the trip. Dress requirements are especially important to protect the students as well as LeaderTreks’ relationship with our host partners. It is all clearly outlined in the Standard of Conduct. Please direct any questions to your trip leader.
